CARES Act Provider Relief Fund: Phase 3 Portal Open

The Department of Health and Human Services opened a portal this morning to allow interested parties to apply for the Phase 3 General Distribution from the CARES Act Provider Relief Fund.  The links below will direct you to application instructions and the portal.  Please note, the Phase 3 distribution expands eligibility for funds and allows for those who missed a prior application deadline, were previously ineligible due to a tax id change, or initiated operations in 2020, to submit an application.

Providers may be eligible regardless of whether they were eligible for, applied for, received, accepted, or rejected payment from prior Provider Relief Fund distributions.

Please note, before you can start the application process, you will be required to apply for an Optum ID login and password. Instructions on how to apply for this ID are in the portal.  We recommend that you consult your accountant to complete the Provider Relief Fund application (PRF), as it requires tax, revenue, and expense information.
